Background Information for Application
Requirements, Definitions, Policies, and Procedures for Educator Professional Conduct are subject to change by the Ohio Department of Education
The Ohio Department of Education is committed to ensuring that all P-12 students receive instruction from educators who can provide a safe, supportive, and healthy school environment. By Ohio statute, those persons who have demonstrated “unbecoming conduct” may not be licensed to teach in Ohio’s schools. The Office of Professional Conduct at the Ohio Department of Education is responsible for raising awareness among pre-service educators for what unbecoming conduct is for educators and has provided a list of offenses that may keep a prospective educator from gaining licensure.
Effective July 1, 1987. The Ohio Department of Education requires that all college/university students admitted to Educator Licensure Programs in Ohio be deemed of Good Moral Character (Rule 3301-21-01 of the Administrative Code).
Antioch University Midwest is, therefore, obligated to require that all Antioch University Midwest students pursuing programs leading to Ohio educator licensure submit to a Bureau of Criminal Investigation (BCI) and a Federal Bureau of Investigation (FBI) review as part of the application process. These background checks must be completed at Antioch University Midwest. The results will be distributed to the Antioch University Midwest licensure office for review. Background checks are only valid for one year. The background checks will also be completed thirty days prior to student teaching.

If you answer YES to any question, attach an explanation to this application. Please include the year of conviction, the nature of the offense and the court where the matter was heard.
